    How to Register for Services


    Make an appointment by calling (314)367-7848 ext 1252.

    Come into any of our 3 locations and meet face to face with an Appointment Clerk, or Front Office Associate

    Registration also includes giving consent for your care and making arrangements to pay for your services. Our providers may not treat you without your consent or the consent of a legal guardian, when applicable.

    **Please be sure to have your insurance card available for registration.**

    Uninsured Patients


    People's Health Centers provides quality care regardless of your insurance status. If you do not have insurance coverage, you may qualify for our sliding scale payment plan. To qualify for this self pay sliding scale, you must bring "proof of income". Examples of documents that qualify for proof of income are listed below:

    Current Paystub
    Child Support or Alimony Statement
    Letter from employer on letterheadwith number of Number of hours worked and monthly income
    Letter from Priest stating patient is currently unemployed
    Official Food Stamp Award Letter(hand written letters will not be accepted)
    Official Social Security/ Disability Award letter
    Unemployment Statement
    Full time student (12 hrs undergraduate, 9 hrs Graduate)
    Federal Tax return Forms- 1040

    **Must present a Photo ID, Insurance card, Copay, and/or Proof of Income current within 30 days of scheduled appointment. Your $32.00 copay is due at time of visit, and please arrive 30 mins prior to scheduled appointment time.

    Emergency Medical Visit-.......$85 copay
    Podiatry-Without Proof of Insurance/Employment/Unemployment.......$50 copay

    Prescription Refills


    Prescription refills can be ordered by calling (314)367-7848 ext 1228, and speaking with a nurse. Please be sure to have medication order numbers to be filled along with prescribing Physicians name.
